Click on the “Downloadable Form” above. This form is usually all we need on your first visit.
Download and print it out, take your time filling it out in the comfort of your home before the date of your visit.
Of course, if you’d rather have something to do in our waiting room, we’ll have these same forms on hand there as well. Just plan to be there about 15-20 minutes earlier than your appointment time, to make sure you have adequate time to provide this information to our staff.
